To bring it back to the OP, and the pros and cons of surgery... I feel like this is quite similar to the knee issues. There are tendons involved, and bone, and the traditional surgery, which is open surgery, has so many possibilities for complications, while the newer, minimally invasive surgical option is controversial and has fewer skilled/experienced practitioners...

I don't know if there are longitudinal studies (like 10 years post-op) of this surgery (I have looked before and will do so again) but I wouldn't be surprised if the findings turn out to be similar to the knee surgery findings.

Tendons heal very slowly. But the standard medical guidance is surgery if conservative measures don't show results after 6 months. If tendons heal so slowly, why wouldn't it take longer than 6 months?

Right now, I can walk. If I have the surgery, I won't be able to walk for probably 6 months. So why hurry?

So my current approach is to change the angle of my heel (with footwear and heel lifts) to move the bone and enthesophyte away from the achilles tendon, while also doing specific kinds of PT, and see if I can get better without the surgery. (so as you can guess, this is why footwear is so important to me!)

My last MRI was in May, so I'm going to ask for another MRI in next spring.

At present, these are the closest to perfect-for-me shoes that I've found (I add a heel lift) but they need a slightly larger toe box).

The podiatrist that comes to the next town has been well liked. Some would go to see him to look at him. He was quite attractive. When he was close enough to be working on your feet it was easy to see that he was very good with makeup.
He gave me a hard sell on some orthotics that I tossed after two days. It was an out of pocket expense. I tossed them within two days. They were destroying my knees
Also, the makeup was gone. He looked rather ordinary.
Wonder what he got caught doing?

You can make the toe any width you like with a pair of inexpensive shoe stretching gadgets from Amazon.

One if the best purchases I’ve ever made.

I have a set. I used them on the shoes that I shared a photo of above. They do help, but there’s a limit to how much you can reshape a shoe, a better design would help even more.

I have a pair of shoes that has a toe box that is the perfect size and shape. But the heel is too low and they are mules (so can’t accommodate a heel insert). The shoes in the photo above have a great heel cup and sole, but need a wider toe box.

All I want is the toe box of the first pair of shoes with the heel and sole of the second, in one pair. Sigh.

I’m going to need a knee replacement. The arthritis has gotten to the really painful point. I’m only walking short distances and using a cane. Have seen one doctor who offers a robotic technique but there’s a $4,000 copay I’d have to pay. A relative who’s an orthopedist (2000 miles away) suggested I keep looking. So I’ll be investigating other options.

@Mik there are tons of plastic ones but I like the wooden ones. Something like this:

My feet are freakishly narrow. Nike made N shoes for decades and recently quit. It's the same thing with dress shoes not that I wear them. I can only conclude the market is so limited that it got to a point where companies decided it wasn't worth it for them.

As a tall slim guy, I have found that it’s easier to find clothing in unusual sizes than it was 20 years ago, but you have to do it online. The corollary is it’s harder now to find unusual sizes in physical stores. I’ve all but given up buying pants and long sleeve shirts (casual or dress) off the rack. They just don’t have anything that fits me. Oh and ‘big and tall’ is just that. Tall AND big. It’s not ‘big OR tall’. So I find nothing in those stores.

@jon-nyc Jon, what do you do about suits or sport jackets? Do you go to a tailor and get custom fitted? I imagine it’s hard to get a good fit either online or off the rack.

I buy bespoke sometimes but also will find some suits in department stores that can be tailored to fit me. I get the jacket good (like a 41 long) and the pants have to be taken in dramatically. Basically recut. Fortunately they purposely make the legs very long and untailored on those suits figuring any customer will want it sized perfectly over dress shoes.
